Rep Head Coach

 THIS APPLICATION FORM IS NO LONGER ACTIVE AS OF JUNE 7, 2017 @ 3:00PM

Rep Head Coach Application Form for 2017 - 2018

  1. Respect in Sport for Coaches (different from the Parent RIS program, is good for 5yrs) *Must complete by Dec 1st or you will be removed from the roster*
  2. Criminal Record Check (good for 3yrs) **must be submitted prior to 1st day with Team***
  3. CATT - Concussion Awareness Training Tool ***Must be completed and show proof of certificate prior to 1st day with Team***
  4. The Head Coach will appoint their bench staff, with approval from the EXECUTIVE BOARD MEMBERS.
  5. Deadline date for Rep Applications will be June 7th ,2017

 Recreation Head Coach / Assistant Coach

THIS APPLICATION IS ACTIVE UNTIL SEPTEMBER 8th, 2017 @ 4:00PM

Rec Coaching / Assistant Coaching for REC or REP Application 2017-18

  1. Respect in Sport for Coaches (different from the Parent RIS program, is good for 5yrs) *Must complete by Dec 15th or you will be removed from the roster*
  2. Criminal Record Check (good for 3yrs) ***must be submitted and show proof prior to 1st day with Team*** or you cannot be put on the team roster.
  3. CATT - Concussion Awareness Training Tool ***Must be completed and show proof of certificate prior to 1st day with Team***
  4. Coach 2 Course

  6. Checking Clinic - All Head Coaches for Bantam and above.

 Safety Person

THIS APPLICATION FORM IS NOW ACTIVE UNTIL SEPTEMBER 30, 2017

  • HCSP (Hockey Canada Safety Program) Must have ONE person qualified prior to team entering the ice-surface on 1st day. (expiry 3yrs) 
  • Respect in Sport for Coaches (different from the Parent RIS program, is good for 5yrs) *Must complete by Dec 1st for Rep or Dec 15th for Rec or you will be removed from the roster*
  • Criminal Record Check (good for 3yrs) **must be submitted prior to 1st day with Team***
  • CATT - Concussion Awareness Training Tool ***Must be completed and show proof of completion prior to 1st day with Team***

***IF YOU DO NOT HAVE A QUALIFIED HCSP BY YOUR FIRST ICE TIME YOUR TEAM CANNOT ENTER THE ICE SURFACE*** NO EXCEPTIONS

  2. Criminal Record Check (good for 3yrs at SAMHA) ***must be submitted and show proof prior to 1st day with Team*** or you cannot be put on the team roster.
  3. CATT - Concussion Awareness Training Tool ***Must be completed and show proof of certificate prior to 1st day with Team***

 On-Ice Helper

THIS APPLICATION FORM IS NOW ACTIVE UNTIL SEPTEMBER 30, 2017

  1. Respect in Sport for Coaches (different from the Parent RIS program, is good for 5yrs) *Must complete by Dec 1st for Rep or Dec 31st for Rec or you will be removed from the roster* 
  2. Criminal Record Check (good for 3yrs) ***must be submitted and show proof prior to 1st day with Team*** or you cannot be put on the team roster.
  3. CATT - Concussion Awareness Training Tool ***Must be completed and show proof of certificate prior to 1st day with Team***

Please note that ALL On Ice Helpers MUST be approved and registered in the Hockey Canada registry to be eligible to go on the ice. The On-Ice Application Form is MANDATORY and if not completed and approved by SAMHA Administrator: ROY SAKAKI you (CANNOT ENTER THE ICE SURFACE).

Below are the links to the Mandatory courses a volunteer must complete along with the appropriate volunteer application forms for which you wish to apply for. These mandatory courses are regulated through policies and procedures by Hockey Canada and BC Hockey. The expiry dates and mandatory completion times are set through BC Hockey. Make sure you are aware of the courses required and the time frames to have them completed by.

 

NEW Guidelines for Criminal Record Checks Regulation 3.07

All BC Hockey Junior and Minor Team Officials must submit a Criminal Record Check prior to any team participation in practices and games. A satisfactory CRC is valid for three (3) playing seasons and then must be renewed thereafter.

The Concussion Awareness Training Tool (CATT) is a required certification for all Team Officials to have completed before participation in any practice or games.

The HCSP training program provides participants with information they need to promote and implement improved safety, risk management and injury-prevention practices in hockey. Hockey Canada has adopted the safety program as a mandatory program to show its commitment to improving the safety and enjoyment of the game.
